Pre-Screen Covid-19 Self Checklist

If you answer YES to any of these questions you will need to reschedule your appointment until all symptoms have passed!

1. Have you tested positive for Covid-19 in the past 14 days?

2. Have you experienced any of the following symptoms in the past 14 days?

fever
dry cough
body aches
sore throat
shortness of breath
loss of taste and/or smell
diarrhoea

3. Are you or your immediate contacts in a high-risk category?

The CDC defines the following vulnerable individuals as being at high risk:

people aged 65 years and older
people of all ages with underlying medical conditions, this includes:
- chronic lung disease or moderate to severe asthma
- serious heart conditions
- immunocompromised
- severe obesity
- diabetes
- chronic kidney disease and/or undergoing dialysis
- liver disease
